This article walks you through everything you need to do before your child can have their first Hazel Health visit from home: filling out the eConsent form, creating your parent account, and attaching your child (or children) to that account. It also shows you how to send us a message and how to review the visits your child has had with Hazel. Every step below is also demonstrated in this video.
Before you begin
Setting up takes about 5 minutes. It helps to have these ready:
- Your child's information β name, birthdate, and any allergies, medications, or health conditions
- Your child's insurance card, if they have one (you can take a photo of it during sign-up). If your child doesn't have insurance, you can still sign up β Hazel is always at no cost to your family.
- Your mobile phone β we'll text you a verification code
- Your child's Student ID number β needed if you add more children to your account later

Step 1: Find your school district's sign-up page
Each school district that partners with Hazel has its own sign-up page. There are two ways to get there:
Option A β from a link your school sent you. If your school or district shared a Hazel link (in an email, flyer, or newsletter), that link takes you straight to your district's page. Skip ahead to Step 2.
Option B β from hazel.co. Go to hazel.co, click Sign Up in the top menu, and choose your state from the dropdown.
After you choose your state, you'll see the list of participating school districts. Find your child's district and click Sign Up next to it.
You'll land on your district's Hazel page. It explains the services available at your school, when they're available, and answers common questions β including why we ask for insurance information. When you're ready, click Sign Up Now.
Can't find your district? Scroll to the bottom of the sign-up page and use the "Not finding your school district?" link.
Step 2: Fill out the eConsent form
The Health Services Consent Form gives Hazel permission to care for your child at school and at home. Fill out one form for each child. Here's each section, in order:
Student information
Enter your child's first and last name and their birthdate.
Then answer the yes/no health questions about your child:
- Allergies β medication, food, and seasonal/environmental
- Current medications β whether your child takes any medication today
Over-the-counter medication permissions
Next, you decide which common, age/weight-appropriate over-the-counter medications a Hazel provider may recommend or administer to your child at school β things like Tylenol, Advil, throat lozenges, and Benadryl. You have three choices:
- Approve all β allow the whole list
- Decline all β allow none
- Let me choose β approve or decline each medication one by one
Health history
Answer yes or no for a list of health conditions β asthma, anxiety, ADD/ADHD, diabetes, seizure disorders, past surgeries, and others. This helps Hazel providers care for your child safely.
You'll also be asked whether your child has a primary care doctor. If they do, Hazel uses this to coordinate care β and if you provide the doctor's fax number, Hazel can send them a summary after each visit.
Insurance information
Answer whether your child has medical insurance. If yes, upload a photo of the front and back of the insurance card β or click "Enter details manually" if you don't have the card handy.
Why does Hazel ask for insurance if visits are free? Hazel has partnered with your school so there is no cost to your family. Hazel bills your insurance to cover the cost of the visit. You may receive an Explanation of Benefits (EOB) in the mail afterward β an EOB is not a bill; it's just a record that a visit occurred. No action is needed.
What if my child doesn't have insurance? You can still use Hazel at no cost. Just answer "No" β Hazel may verify uninsured status after sign-up.
Parent/guardian information
Enter your name, mobile phone number, and email. You can also add a second parent or guardian β helpful if either of you may need to join or discuss your child's visits.
Use a mobile number you can receive text messages on β it becomes your account login and is used for the verification step below.
Authorization and signature
Read the Authorization and Privacy Policy, check the box confirming you give permission for your child to receive care from Hazel providers, and type your full name in the signature field. Then click Submit Consent.
Step 3: Verify your phone number
After submitting, we'll offer to text a verification code to the mobile number you entered. Choose your number, click Send Code, and enter the code when it arrives. (You can also choose Verify Later, but verifying now makes signing in easier.)
You'll then see a confirmation that your consent form was received.
From this page you can:
- Sign Up Another Student β fill out a consent form for another child, or
- Sign In To Your Account β continue to your parent account (or create it, if this is your first time)
Step 4: Create your parent account and sign in
Your Hazel account uses your mobile phone number and a password. If you're new to Hazel, follow the Create Account prompts to confirm your details and set a password; afterward, sign in with your phone number and password.
Step 5: Attach your child (or more children) to your account
The child whose consent form you just submitted is connected to your account automatically. To add another child, open the Children section of your account home page and click Add Child. You'll need three things:
- Your child's school district (searchable)
- Your child's Student ID number
- Your child's birthdate
If the information is correct but you still can't add your child, call Hazel at 1-800-764-2935 (1-800-76-HAZEL) and we'll help.
Your account home: what you can do
Once signed in, your home page shows everything in one place:
- Visits β upcoming and joinable video visits for each of your children. When a provider is ready, the visit shows a Join Video button.
- New Messages β a notice whenever a Hazel provider has sent you a message about one of your children.
- Children β every child attached to your account. Select a child to see their previous messages, visits, and instructions.
Sending Hazel a message
Have a medical question about your child? Select the child from your Children list and click Contact Doctor. You can send a message to a Hazel doctor, and if needed we can schedule a time for a doctor to call you to discuss your child's health in more detail. Replies appear in the same place, and you'll see a New Messages notice on your home page.
Important: messaging is not for emergencies. If this is an emergency, please call 911.
Reviewing your child's visits
Each visit β at school or at home β appears on your home page and on your child's page. Open a visit to see its details, join the video when a provider is ready, and view messages related to that visit.
