Students must have a consent form completed by a guardian to use Hazel’s services at school. Teachers and staff can help encourage families to return this consent form, even if they would like to opt-out of care to help confirm that we have reached all families.
If a student isn’t feeling well, teachers and staff can refer them to the school health office like normal, where the school nurse or health staff can determine whether to initiate a Hazel visit.
If you notice a student showing changes in their behavior, moods or have experienced emotional distress, you can refer the student to Hazel for a behavioral health screening and consideration for Hazel's mental health services.
If teachers and staff cannot answer a question from a guardian, the guardian can call Hazel directly at 1-800-764-2935.
