- How do I upload paper consent forms?

Uploading a Consent in the Hazel App

When a student comes to the health clinic with a Hazel consent form, you are able to add it to their profile immediately. Begin by signing into the Hazel app on the iPad.

Tap Find Student.
Search for the student and select the correct student’s profile.
Confirm that the student’s name and birthdate in the app matches the information on the paper consent form. Then tap Update Guardian Consent Form to upload a consent.
Review the approval checkbox at the bottom of the consent, then select either Guardian Approved Service or Guardian Declined Service. Note If you select Guardian Declined Service, you will then have to tap Signed Consent Form before proceeding to step #5.
Tap the camera icon or Take Photo of Page 1 to take a photo of each side of the consent form.
After taking the photo, review the image to ensure that: All edges are visible The photo is clear The photo is in portrait mode (not sideways) If all above items are true, tap Use Photo in the bottom right corner to upload. If needed, tap Retake in the bottom left corner to take a new photo.
Repeat steps #5 and #6 to take a photo of the back / page 2 of the consent form.  If the back is blank or the second page is missing, tap Missing Page to skip uploading page 2.
After taking a photo of the front and back of the consent form, tap Send Consent Form.  The consent form has now been uploaded!

Uploading a Consent During a Visit

If you are in the middle of a visit and need to re-upload the consent or upload a new consent form, please follow the steps below.

While in the visit, tap the three dots in the upper right corner of the iPad screen and select Sign Out.
Tap your name at the bottom of the screen to sign back in using your PIN. If a different user will be signing in, tap the blue Sign In button instead.
Tap Find Student then follow steps #2-8 on pages 1-4 in order to upload a new consent form.
Once the consent form has been uploaded, tap Back in the upper right corner (two times) to return to the dashboard.
On the dashboard under the left column titled Visits, tap on the student’s visit that states Visit in progress.
Tap the orange Resume Visit button to rejoin the video. The consent form has now been updated in the student’s profile and can be reviewed by Hazel’s team!
