Enroll 18+ students in Hazel's Teletherapy Program

Student Referral 

Students who are 18 years or older can request a referral by talking to their school counselor. 

The referral must be submitted by a referrer. 

1. On the mental health referral tab select "refer a student" and enter the student's name
2. When asked "would this student like their parent/guardian to be notified?" select "NO"
3.  Enter your contact info and submit the referral Minor consent, continue: 
4. Student fills out the Minor Consent Form, found under the Student's tab
