Visit information is accessible in the Hazel App for two weeks following the visit.
For information more than two weeks after a visit, you can ask the family for a copy of the discharge summary.
If you cannot connect with the family, please send a request via the General Chat in your Hazel App on the Hazel iPad or the Hazel Health Dashboard.
Include the student’s name, date of birth, and visit date.
We will send you the requested discharge summary via a secure message.
