Our school needs more paper consent forms. Can you send them to us?

To collect the most accurate information and improve family communication through the Hazel Parent Account, we recommend that families use the eConsent form to consent their students. Completing the eConsent results in quicker access to Hazel’s services and reduces the burden of mailing or scanning the form. 

Families can access the eConsent form from your district’s Hazel Health website. If unsure of your URL, please click here to search for your district. Click Sign Up to be redirected to your Hazel District URL.

Districts can also access PDF versions of consent forms from their account manager or Brandfolder link.
