Guardian Consent Form

Consents are needed for Hazel to provide services to any student.

The below steps ensure consent forms are completed and submitted correctly. 

Guardians will need to follow the below steps to complete an eConsent form.

Start	Visit https://www.hazel.co/get-hazel and search the state your student resides in.
Step 1	Find your district, and click "Sign Up"
Step 2	A new page will open. Click "Sign Up Now"
Step 3	Complete the student and guardian fields on the consent form. Information includes: A brief medical history of your student  Your student's insurance information (if they are insured)  Your contact information Review the Authorization and Privacy Policy
Step 4	Proceed by clicking the checkbox confirming permission for your child to receive Hazel Health Services
Finish	Sign the form by typing your name and click Submit Consent
