To prevent sensitive Protected Health Information (PHI) from being exposed, Hazel Health requires that school staff and families only communicate with us about a student’s or child’s specific case through secure channels.
Initiators, referrers, and other school staff
- The Hazel Web Dashboard: sign into the Hazel Web Dashboard at my.hazel.co/school; once signed in, you can message Hazel Health directly through the question mark icon on the bottom right of the screen.
- Calling Hazel: call Hazel at 1-800-764-2935 to speak directly with a Hazel Health support teammate
- Calling Hazel Health Account Manager or Customer Success Manager: call directly for assistance
Families
- Their guardian account: families can sign in via my.hazel.co/patient; once signed in, they can message the therapist directly by clicking into the visit and starting a chat.
- Calling Hazel: call Hazel at 1-800-764-2935 to speak directly with a Hazel Health support teammate
