Navigating "Discharged" Reasons on the Referral Dashboard

You can now hover over any “Discharged” status under the HEART Program Status column to better understand why your student has been discharged from our program. 

Here are the reasons that may be displayed when you hover your cursor over “Discharged” 

Administrative Discharge

Patient abandoned care program

Attendance Violation - No response from guardian after multiple reschedule attempts. Student may re-enroll at a later date

Attendance Violation - Student has cancelled 2 consecutive or 3 total visits less than 24 hours in advance. Student may re-enroll at a later date.

Multiple missed intake appointments

Guardian retracted consent

Therapist determined clinically inappropriate

Opted out of therapy mid care program

Student refused to attend

Family is accessing services elsewhere
