Creating a student account

Please follow the steps below to create your student account (for students 18+ only)

Step 1

Navigate to my.hazel.co/patient and click the "Sign up now" button

Step 2

Enter your phone number (one that can receive text messages) and click "send SMS code" then enter the code on the page that pops up. 

Step 3

After you've verified your phone number, choose STUDENT and fill in your student info / birthday. 

NOTE: If you choose "parent" we will not be able to switch your account type and you will need to start from the beginning using a new phone number. 

Step 4

The bottom of the page will open up after you put in the info above, this is where you will enter your name, create a password, and read our terms and privacy policy.

Click "create account" when you're done and you will be taken automatically to your new account!

NOTE: You will need to let your therapist know when your account is created so that they can give you access to all of the visits you have scheduled with them.
