You have submitted a referral for your student, now what? Below you can find an outline of the student walk through after a referral has been submitted.
After the referrer submits a referral, a student will appear in the referral dashboard with a status of “Scheduling.” Note that the student may already show up as “Active” if the student is already enrolled in HEART.
Hazel will then reach out to the guardian to gather Opt In. The Opt In Status will update with the number of call attempts Hazel has made.
Once Hazel is able to connect with the guardian and gather Opt In, Opt In Status will update with a green checkmark.
If the guardian decides to Opt Out, Opt In Status will show a red X. Placing the cursor over Unable to Enroll will display the reason the guardian chose to Opt Out.
After Hazel gathers Opt In from the guardian, Hazel will gather consent if it has not already been completed. After consent is gathered, a green check mark will appear under Consented.
Once consent is completed, Hazel will schedule an appointment. The appointment date, time, and site will show in Next HEART Visit and the HEART Program Status will change to ‘Active.’
As the student moves through the HEART program, Next HEART Visit and Completed HEART Visits will update.
HEART Program Status will update to ‘Discharged’ once the student has completed the program and is discharged from HEART.
