Overview
The referral dashboard provides real time updates on the status of a referral with additional features that provide more visibility into the enrollment process after a referral is received.
How to access
The referral dashboard automatically appears after submitting a referral. You can also access the dashboard by clicking on Mental Health Referral in the menu on the left or going to https://my.hazel.co/school/referral.
Checking the referral status
The referral dashboard shows a list of all referrals that you have submitted in order of submission date. Each referral will have the following information:

HEART Program Status |
A referral can have one of four program statuses: Scheduling: Hazel has received the referral and is working with the student's family to schedule the intake visit Active: The student is actively enrolled in the HEART program Discharged: The student has completed the HEART program Unable to enroll: Hazel was unable to schedule a session. A pop up will appear when hovering over the label that describes the reason Hazel was unable to enroll a student. |
Opt In Status |
After a referral is submitted, Hazel reaches out to the guardian to confirm if the guardian wishes to opt in or agree to have the student participate in HEART. = guardian has opted in or agrees to have student participate in HEART = guardian has opted out or declined to have student participate in HEART This column will also display the number of call attempts made by Hazel staff to get a guardian’s opt in. |
Consented | = Student has a valid consent on file to participate in HEART |
Next HEART Visit | The date, time and location (At School, At Home) of the next HEART visit |
Completed HEART Visits | Number of HEART visits completed in current HEART care program |
Understanding why Hazel was unable to enroll a student
If Hazel was unable to enroll a student, the referral will have a status of 'Unable to Enroll' with a hover text that displays one of the following reasons:
- Receives services already
- Alternate intervention recommended
- Uninterested in services
- Uninterested in telehealth
- Prefers services through school
- Prefers services through community partner
- Declined without reason
- Unaware of referral
- Seeks long term services
- Declined to consent
- Seeks services sooner
- Failed completing enrollment tasks
- Unable to contact
Filters
You can filter the list of referrals by school, status, referred by, and student name. Clicking on the student name allows you to filter the list to show all referrals for that student or you can jump straight to the student profile.
